Can anyone shed light on where this all began? Of course there is the Metallica logo, or Death Angel logos that had some form of symmetry.

Is there any earlier examples, even out side of the metal genre?

Was Christophe Szpajdel the first to push it to the the extremes?

The earliest example I know of would be Faithful Breath, formed in '67, released their first album (already with that logo) in '74

You can see it probably arrived in music via the psychedelic artwork of the 60s. Band names or album titles were sometimes rendered into a symmetrical shape. Just not consistently between releases it seems, since they didn't use band logos there (if they even had one at all).

For example, Vincebus Eruptum & New! Improved! by Blue Cheer, and Aoxomoxoa by Grateful Dead:
