And my band would have been #29 to use that cover for our first demo. Glad I found the other 28 that also decided that this image, and for whatever reason thought it was original, haha.

Bathory's Blood Fire Death and Ukrainian outfit Чиста Криниця's Симфонія Життя. I also recall seeing the same piece of artwork Blood of Kingu used for Sun in the House of Scorpion somewhere else as well, but I can't remember off the top of my head, oh well.

I remember hearing the cover for Rainbow's Difficult To Cure was originally going to be used for Sabbath's Never Say Die. Or was it the other way around?

Not entirely identical cover pictures, but the artwork for Beherit's Dawn of Satan's Millennium has been used on everything from demo covers to fucking books.

Harvester of Souls' Visions of an Apocalyptic Wasteland and Winterthrall's Nightmares for the Sleepless share the same Beksiński piece for their artwork. I recall seeing a couple other bands using this piece as well, but can't remember them off the top of my head.

Well fuck.... haha. Yeah Beksiński has some pretty cool pieces, seems to be quite popular as well amongst metal bands if that above link is any indication.

One of my favourites from him used on a cover. Wish BoK didn't go dun goof it up with that banner/logo on the side, but oh well. Great album too, at that.
