Contrary to popular belief, this is neither the last rehearsal with Dead, nor was it recorded in 1991 - it is actually a rehearsal recorded in 1989.
Dead sent out at least one copy bearing the title "Bloody Rehearsal '89". That version of the recording has eight tracks and the original track running order, including banter and noise. The picture disc release has only seven tracks, "Carnage" and the banter and noise is edited out. It also has a rearranged track running order.
These seven tracks were also featured on the "In Memorium" bootleg CD re-release, as well as on several other bootlegs. Usually they are erroneously credited as recordings from 1991.
