The cover is actually a detail from the "Among the Living" album artwork. The waving man is supposed to be Randall Flagg, a character from Stephen King's "The Stand".

Oidivnikufesin (N.F.V.) is "Nice Fucking Video" in Anthrax-speak (backwards).
